I've seen it on invisionfree or phpbb forums, not sure which one. Basically, you can create your own BBCode. You select tags and what HTML the tags are supposed to be replaced with. Oh, and you choose the image to be displayed in the posting area to insert the tags.

The pro SMF forums also have this mod installed. Its pretty cool. :)
