How do I set up a javascript page?!

Like Agent Moose did with this code

How would I set a page like that up? I wanted to edit a few things with that code (I have permission from AM to do so) but it didn't work when I put the info from that page in a webpage editor document...

Could someone help with this?

I requested Agent Moose tell us how to do this, or make a code to easily make a page, and he hasnt gotten back to me on it.

However, that page was made at google pages, which is not accepting sign-ups anymore.

Well in that code it's a support ticket. Where you click a tab and it brings up a form so you can report problems.

I was going to make a "Sign Up for Affiliates" form by editing a few things with it. But that won't go in a webpage.
