Custom skin of forum gone

Hi folks!  I suddenly encountered this strange problem.  The custom skin I installed in my forum is replaced by default skin.  And I'm not being able to re-install the custom skin.  I hope my forum hasn't been hacked or something .....    Can anyone please let me know what's happened and how I can fix it ....  please.

Many thanks for this link sybil.  I'm having exactly the same problems .. and my custom tabs are also gone.  If the problem is only with some forums of server 1, would appreciate if you kindly forward my forum URL to SMF too as one of the affected ones.
