Strange.    I had it on my test site for at least 3 weeks before putting the current one on it.   It never once disappeared (and I see no way that could just happen)    I know Moose had it on one of his for a long period of time too.

I can find no reason for it to just disappear out of the blue, when it has worked fine for weeks.    The only thing I can think of is to try it without any codes in the header/footer.   See if it works okay for you that way  (it should)     Put one code back in at a time and see how it goes.   Go through your codes in your header completely and check to see if something is left open/missing/etc.   Honestly don't know what else could be causing it -  it has never disappeared on me  (none of the stylesheets have)   I have it on one of my test sites right now.

Best bet would be remove them all.
Then try each one, one at a time.   See if you can pinpoint which one is doing it.   OR if its a combination of a couple.  It could be something like a missing closure to something...    for example if you put  <style> and dont close it you'll have NO forum at all..  only the header will be there.
