If you mean like i runescape private server status, go to runelocus.com and look for status checker on the left.

 Do you want a server status page or image? I do not know how to make a page, but like they said go to runelocus.com and go to >Status Check (under Tools) Type in the IP of your server and Port > Then click Check Status. It will bring you to a new page with the code for you to use. The image will say Offline or Online depending on the servers status. Also I can just give the code below and edit your Ip and Port into it.

IP = put your server IP

PORT= put the port to your server
